About Spine Surgery

Spine surgeons are physicians who receive special training in the diagnosis and treatment of the spine which includes problems with the neck (cervical spine), mid back (thoracic spine), and low back (lumbar spine).

We provide specialized care for adults as well as children.  Our primary office has x-ray services readily available to complement your visit with the physician.

Appointments/Office Hours

To best serve you, office visits are scheduled by appointment only.  Our primary office is located in downtown Louisville in the Gray Street Medical Building, Suite 601.  You can make an appointment online at our website or by calling 502-585-2300 from 8:30 am to 5:00 pm.  We also have satellite locations in Jeffersonville and Corydon, Indiana, and LaGrange, Kentucky.

Our practice observes most traditional holidays.  In addition we are closed Oaks Day (the day before Derby) and the Friday after Thanksgiving.

If you are unable to keep you appointment, please call the office at your first opportunity.

Parking

The first five floors of the Gray Street Medical Building are devoted to parking.  Our receptionist will validate your parking ticket for up to one hour.  Each satellite office provides free parking in lots adjacent to the buildings.

Hospitals

Our physicians primarily use Jewish Hospital in Louisville and Clark Memorial Hospital in Jeffersonville.  They also have privileges at several other facilities including Norton Hospital, Audubon Hospital, Suburban Hospital, Baptist Hospital East, Baptist Hospital Northeast in Kentucky and Floyd Memorial Hospital in Indiana.

Disability Forms/Evaluations

For disability ratings, we defer to practices that specialize in these evaluations.

We prepare disability forms for our patients in the order in which they are received.  We charge $10.00 to complete each form.  Requests from State Disability Determination Departments are paid by the requesting state.

Emergencies

If you feel your condition requires immediate attention, please call 911 or go to the closest emergency room.  You may call the office number at any time and we will return your call as quickly as possible.  Depending on the nature of the condition and the time of day, you may be instructed to go to an emergency room or come to the office, or you may receive advice that may remedy the problem.

Refills

Medication taken on a routine or chronic basis can frequently be refilled by telephone.  When your medicine is running low, please call your pharmacy.  The pharmacist will then contact us for authorization.  Allow 24 hours for the entire process to be completed.  Refill requests are not called in to the pharmacy until after 4:00 pm each day.

We normally decline refill requests made after office hours or on weekends when the patients' medical records are not immediately available.  If it appears your medication will run out over a weekend, please be sure to call your pharmacy no later than Friday morning so we can review the records and respond to the pharmacy before the office closes for the weekend. 

No Show/Cancellation Policy

Patients who have multiple No-Shows and/or cancellations will receive a warning.  If an appointment is missed following the warning, no further appointments will be scheduled.

Insurance/Billing

As a service to you, we will file your insurance claims for office services and surgery.  We file electronically in most cases, or use a standardized health insurance claim form which is accepted by most major carriers.  If your insurance company has its own claim form, please provide us with copies.

If your insurance requires a referral from your primary care physician, you are responsible for obtaining that referral before your office visit at Spine Surgery, PSC.

We do accept assignment on Medicare and also file Medicare supplemental claims.

Please remember that professional services are rendered and charged to you, not your insurance company.  You are responsible for any charges not covered by insurance.  If you do not have insurance coverage or do not bring the required information for filing, you will be required to pay at the time of service.  We also ask for copays at the time of service.  If you are unable to pay all charges at the time of service, we will assist you in establishing a reasonable payment schedule.

An account is considered past due after 30 days and delinquent if no payment has been made after 90 days.  Delinquent accounts may be sent to a collection agency if no attempt has been made to arrange an acceptable payment schedule.
